III. COLLABORATION WITH OTHER AGENCIES
1. The Bureau and E. Newton Smith Public Health Center STD Clinic continues to refer clients to each on a regular basis. During the first quarter seven (7) clients/contacts were referred to E. Newton Smith Public Health Center with referral cards through the Bureau’s one on one outreach efforts.
The Bureau and E. Newton Smith Public Health Center STD Clinic continues to refer clients to each on a regular basis. During the second quarter seventy–eight (78) clients/contacts were referred to E. Newton Smith Public Health Center with referral cards through the Bureau’s one on one outreach efforts.
During the first two (2) quarters eighty–five (85) clients/contacts were referred to E. Newton Smith Public Health Center with referral cards through the Bureau’s one on one outreach efforts.
2. As of August 21, 2002 the Bureau and E. Newton Smith Public Health Center are continuing to network and collaborate to reduce the spread of HIV/AIDS, Syphilis and other Sexually Transmitted Diseases. E. Newton Smith Public Health Center continues to provide for the Bureau laboratory quality assurance, process field blood, mail HIV/AIDS blood samples to the State Laboratory, Syphilis lab work is also conducted at the Health Department and disposal of all bio--hazardous materials. The Bureau is responsible for providing trained counselors for HIV Antibody and Syphilis Pre and Post Test Counseling; and documentation as required by E. Newton Smith Public Health Center; continuous quality improvement. Also, the Bureau notifies E. Newton Smith Public Health Center and DHHS Region V Office immediately upon receipt of positive test results. After a medical record search through the State and E. Newton Smith Public Health Center, the Bureau arranges for the client to be counseled jointly with the DHHS’s Region V DIS. Later with the assistance of the DHHS Region V DIS, the Bureau completes a Communicable Disease Report Card and a HIV Surveillance Form, which is submitted to E. Newton Smith Public Health Center. Contact tracing is conducted by NC Department of Health and Human Services Region V along with the Bureau’s staff. All positive HIV/AIDS and Syphilis clients are taken to E. Newton Smith Public Health Center for appropriate treatment and/or care. The Bureau mails the Post–Test counseling section of the HIV Scannable Form to the State Lab under the guidance of the NC DHHS Region V Office. The Syphilis test results for individual targeted clients are sent to Mrs. Lynne Sampson at NCSEP in Raleigh, NC. Also the Bureau has secured filing cabinets for all files and test results, provides condoms and safe/safer sex literature and intends to do client satisfaction surveys.
3. The North Carolina Department of Health and Human Services Regional V Office has networked and collaborated with the Bureau on a consistent basis, building the infra structure of the Bureau’s Non–Traditional Counseling, Testing and Referral Sites. The Regional Office has also been very instrumental in training and providing technical assistance to the Bureau’s Staff. This has been spearheaded by Mrs. Connie Jones, Mr. Dwight McDowell and Mr. Wayne Williams. Also, Mrs Marti Eisenberg–Nicolayson of North Carolina HIV/STD Prevention and Care Branch has worked cooperatively with the E. Newton Smith Public Health Center and the NC DHHS Regional Office to bring together this joint collaboration.
4. The Bureau’s Medical Director, Carl Foulks, MD, has issued Standing Orders for the Bureau concerning its OraSure® HIV Antibody Testing. OraSure® HIV Antibody Testing is being conducted in conjunction with the HIV/AIDS and Syphilis blood work.
5. The Cumberland County Mental Health Department, E. Newton Smith Public Health Center, NC DHHS Region V Office and the Bureau continues to network and collaborate.
6. The Bureau has a referral system with numerous homeless shelters and adult care facilities for clients with substance abuse problems, those living with HIV/AIDS and those highest at risk for contracting HIV/AIDS, Syphilis and other Sexually Transmitted Diseases.
7. The Bureau continues to network and collaborate with the Region V Community Planning Group.
8. The Bureau’s Community/Street Outreach Educators continues to attend the regularly held NC Outreach Network Training events.
9. The Bureau’s Nontraditional Testing, Counseling and Referral Main Office located at 110 ½ Gillespie Street, and the Nontraditional Testing, Counseling and Referral Satellite and Outreach Office located at 221 Old Wilmington Road has increased access to HIV/AIDS, Syphilis, other Sexually Transmitted Diseases Prevention, Counseling, Testing and Referral services to high risk targeted populations in Cumberland County, especially IDUs and other substance users, MSM of African Descent (closeted bisexual/down low men), women of childbearing age and their partners, including adolescents females and their partners of African Descent. This is due to collaborating with the local faith, business community, health/mental health departments, NC DHHS Regional V Office and most importantly the “gatekeepers” of the targeted populations. These services are provided during nontraditional hours and on weekends.
IV NARRATIVE PROGRESS TOWARD GOALS
A. GOAL AND OBJECTIVES
Goal: The Cape Fear Regional Bureau For Community Action, Inc. (The Bureau) will work through this project to increase high risk hard to reach targeted populations accessibility and utilization of nontraditional HIV/AIDS, Syphilis and other Sexually Transmitted Diseases prevention, counseling, testing and referral services; and increase the number of high risk hard to reach targeted populations being tested for HIV/AIDS, Syphilis and other Sexually Transmitted Diseases in Cumberland County, North Carolina.
Objective A: By June 30, 2003 conduct two hundred (200) sessions of street/community outreach to market the availability of nontraditional testing, counseling and referral services.
During July through September 2002, first quarter, the Bureau conducted fifty–four (54) street/community outreach sessions. The total targeted population reached during the first quarter was four thousand fourteen (4,014). This targeted population includes IDU and other substance abuse users, MSM, particularly closeted bisexual and down low men, commercial sex workers, repeated STD infected individuals, high risk women of child bearing age and their partners, including adolescent females and their partners, homeless and gang members of African Descent.
From October through December 2002, second quarter, the Bureau conducted sixty–six (66) street/community outreach sessions. The total targeted population reached during the second quarter was three thousand three hundred–nine (3,309). This targeted population includes IDU and other substance abuse users, MSM, particularly closeted bisexual and down low men, commercial sex workers, repeated STD infected individuals, high risk women of child bearing age and their partners, including adolescent females and their partners, homeless and gang members of African Descent.
During the first two (2) quarters, the Bureau conducted one hundred–twenty (120) street/community outreach sessions. The total targeted population reached during the first two (2) quarters was seven thousand three hundred twenty–three (7,323). This targeted population includes IDU and other substance abuse users, MSM, particularly closeted bisexual and down low men, commercial sex workers, repeated STD infected individuals, high risk women of child bearing age and their partners, including adolescent females and their partners, homeless and gang members of African Descent.
Objective B: By June 30, 2003 OraSure® HIV Antibody Test and Pre–Counsel one hundred fifty (150) hard to reach targeted populations.
During the first quarter, July through September 2002, the Bureau OraSure® HIV Antibody Tested and Pre–Counseled sixty–four (64) hard to reach targeted individuals, during non-traditional hours and weekends.
During the second quarter, October through December 2002, the Bureau OraSure® HIV Antibody Tested and Pre–Counseled seventeen (16) hard to reach targeted individuals, during non-traditional hours and weekends.
During the first two (2) quarters, the Bureau OraSure® HIV Antibody Tested and Pre–Counseled eighty–one (80) hard to reach targeted individuals, during non-traditional hours and weekends.
Objective C: By June 30, 2003 Post–Counsel OraSure® HIV Antibody Test seventy–five (75) hard to reach targeted populations.
During the first quarter, July through September 2002, the Bureau Post Counseled OraSure® HIV Antibody Tested sixty–two (62) hard to reach targeted populations, during non-traditional hours and weekends.
During the second quarter, October through December 2002, the Bureau Post Counseled OraSure® HIV Antibody Tested fifteen (15) hard to reach targeted populations, during non-traditional hours and weekends, during non-traditional hours and weekends.
During the first two (2) quarters, the Bureau Post Counseled OraSure® HIV Antibody Tested seventy–seven (77)) hard to reach targeted populations, during non-traditional hours and weekends.
Objective D: By June 30, 2003 through traditional HIV Antibody Testing (blood work) one hundred (100) hard to reach targeted populations.
During the second quarter, October through December 2002, the Bureau through HIV Antibody blood work Pre-Counseled and Tested one hundred twenty–three (123) high risk targeted individuals, during non-traditional hours and weekends.
Objective E: By June 30, 2003 Post–Counsel traditional HIV Antibody Testing (blood work) fifty (50) hard to reach targeted populations.
During the second quarter, October through December 2002, the Bureau through HIV Antibody blood work Post Counseled ninety–five (95) high risk targeted individuals, during non-traditional hours and weekends.
Objective F: By June 30, 2003 through phlebotomy (blood work), Pre–Counsel and Test one hundred (100) high–risk individuals for Syphilis.
From October through December 2002, Second Quarter, the Bureau Pre–Counseled and Tested one hundred twenty–five (125) high–risk individuals for Syphilis through phlebotomy (blood work), during non–traditional hours and weekends.
Objective G: By June 30, 2003 Post–Counsel through phlebotomy (blood work) fifty (50) hard to reach targeted populations for Syphilis.
From October through December 2002, Second Quarter, the Bureau Post–Counseled ninety–six (96) high–risk individuals for Syphilis, during non–traditional hours and weekends.
Objective H: By June 30, 2003 provide prevention case management services to seventy (70) clients to asset in clients’ risk factors and facilitate behavior changes.
The Bureau understands that testing alone will not modify or change behavior, that clients need education with workable tools tailored to their life and lifestyle in order for effective behavior changes to take place. Each client is given Pre–Test Counseling and risk assessment. From the assessment a determination of needed behavior changes is noted, for example: from the need to use a condom, stop using dirty syringes, or maintaining a single partner relationship and seeking rehabilitation for alcohol/drug related problems. A risk reduction plan is developed during the Post Test Counseling session with clients to assist with positive behavior changes.
During the first two (2) quarters, July through December 2002, one hundred seventy–two (172) high risk, hardest to reach targeted individuals have been Pre Test Counseled and Post Test Counseled for the OraSure®/Blood Work HIV Antibody and ninety–six (96) were Pre and Post Test Counseled for Syphilis by the Bureau. During the Pre Test Counseling session risk assessment and interpersonal prevention counseling was conducted. During the Post Test Counseling session individuals were notified of their respective HIV Antibody and Syphilis Test results and recommended for followup testing within three (3) month period. This followup testing is to cover any possible recent exposure from a six (6) week to twelve (12) week period. Risk reduction plans are suggested to each agency client for his or her respective risk factors with workable options for reducing their risk for exposure to HIV/AIDS, Syphilis, other Sexually Transmitted and Communicable Diseases.
Of the hundred seventy–two (172) high risk, hardest to reach targeted individuals Post Test Counseled by the Bureau for OraSure®/Blood Work HIV Antibody and Syphilis, ninety–nine (99) of these clients have shown significant progress. The client’s progress has been annotated in the Bureau’s file. Periodical followup will be conducted on all Bureau’s clients to ascertain their progress, relapse and need for referral for other required medical or psychological services. The current Bureau’s Client Progress Notes vary from progressive attitudes toward substance abuse recovery by attempting behavior modification through the medium of environmental change and spiritual influence to developing options for enhancing safer/safe sex control by maintaining a monogamous relationship and greatly decreasing non–injectable drug usage and recreational drugs/alcohol.
Objective I: By June 30, 2003 conduct one (1) HIV/AIDS, Syphilis and Sexually Transmitted Diseases Block Party and Rally to the targeted populations.
On November 27 and 29, 2002 the Bureau conducted a Pre–World AIDS Day Testing Street Corner Outreach Block Party and Rally to the various targeted populations. Seventy–nine (79) were tested for Syphilis and seventy–eight (78) were HIV Antibody through phlebotomy (blood work). One on one street outreach was conducted to over five hundred (500) persons under windy and chilly conditions. Testing was conducted by Bureau’s staff and assisted by volunteers. During this one on one street outreach, condoms and educational literature were distributed to various targeted individuals. The need for abstaining from unsafe sex and using drugs was also discussed with contacts, while conducting street outreach by staff and volunteers. Many sexually active individuals received condoms for safer sex. Injectable drug users were educated about cleaning their drug paraphernalia. Persons with substance abuse were referred to Cumberland County Mental Health Department and others were referred to E. Newton Smith Public Health Center for sexually transmitted diseases screening.
B. Barriers
There is a need for 24-hour counselors and Disease Intervention Specialists.
The Bureau has no bleach/water kits.
The Bureau has no dental dams.
The need for more condoms.
The need for needle exchange.
The need for more full time staff.
The need for more funds for MSM and MSM/IDU outreach and pre/post testing counselors.
