Prevention Case Management
Prevention Case Management (PCM) is a multi-session, client-centered HIV prevention activity targeting clients with multiple, complex problems and risk-reduction needs. PCM is intended for persons having or likely to have difficulty initiating or sustaining practices that reduce or prevent HIV acquisition, and/or transmission. PCM provides intensive, on-going, individualized prevention counseling, support, and referral assistance. This format is distinct and separate from Individual Counseling.
One of the most powerful key words in the Bureau is “prevention.” If we can prevent the crises and the behavior that created the intolerable conditions in our communities, we can begin to see the quality of life improve within our communities. Our Preventative Case Management seeks to do just that.
Every person that the Bureau serves are receiving some degree of Preventative Case Management. Each person is contacted by our community/street outreach educators and an assessment is made concerning their immediate needs. These consumers are encouraged to take an HIV/AIDS Antibody Test and Syphilis Test. They are Pre Counseled concerning the test and given educational materials. In some cases individuals are given referral cards to E. Newton Smith Public Health Center and/or the Cumberland County Mental Health Department for follow-up professional treatment and care. After taking the test, they are Post Counseled and advised what needs to be done if they tested positive for the HIV Antibody or for Syphilis and a plan for behavior change is developed to reduce the risk opportunity for reinfection for them and for those who tested negative, that they are not infected. Based upon this behavior plan and follow up with the consumers the Community/Street Outreach Educators are able to document positive or negative changes in their behavior patterns as reported to them by the consumer.
Some needs are more pressing upon the client. They may be in need of shelter for the night, a place to live in the long term. Many are encountered that have not eaten in days and are in need of a meal. Others are in need of clothing, while still others have serious medical concerns. All of the immediate needs are addressed while the client received positive encouragement and education on behavior changes to reduce the trauma confronting them at the time or the risk of contracting a sexually transmitted disease.
The Bureau acknowledges that it cannot be effective in its Preventative Case Management without the support of other community services to which these consumers are referred. But the most effective and powerful redirection is to reunite these persons with their family and to assist the family in strengthening its family structure. Everyone is needed to have effective Preventative Case Management.
Preventative Case Management at the Bureau is more than an interview with a client, it is the process of developing a long term positive working relationship with our consumers, so together we may work to insure that negative and risky behavior is reinforced with positive and safer behavior, thereby improving the quality of life for those we serve.
